I just purchased a 2010 CR-V 5DR 2WD EX-L NAVI. No you do not pay periodic fees to use. My delivered map version was 6.81A. The update ,6.92, should one chose to purchase is $175.00 according to my service manual. I also have a 2008 Hyundai Azera which has NAVI and although 3 yrs out of date works great on Interstate Highways. POIs are sometimes wrong but not a show stopper.

The only fee you should run into would be the cost to update the maps built into the system. Updated DVDs with updated road and POI information can easily run $300 or more, but you not feel the need to update the system while you own the car.

The Onstar system offered by GM is the only one I know of that requires a monthly subscription.
