How can we make our 1999 Ford F-350 get better mpg?

We have had it less than six months and my husband loves it but it is killing our bank account. I know there are things you can do to help this out. His birthday is coming up and I would like to buy something for the truck.

Most of the things you could buy would help very little in your overall fuel consumption. I've found very few people who would recommend anything that would be worth the money.

Good driving habits would contribute the most. However, with a vehicle like yours, mileage is never going to be very good.

I don't think anything you buy is going to help. Best thing you can do is watch your driving, maybe run synthetic oil, and pump up the tire pressure. All the other "gadgets" just don't have the payback for what you spend to buy them and install them, or they outright don't work, or they might damage the vehicle.

But synthetic oil could give you 1 mpg maybe and hard tires maybe .5 mpg.

buy your husband a lighter pair of shoes, seriously, these are heavy trucks with heavy duty drive trains and low gearing for pulling, I have a friend who gets the same mileage empty or full, up hill or down. Not much to do that hasn't already been stated here in the previous answers. Make sure it is running well, if it is four wheel drive and you have a 2wd option run it that way, if it is full time you might pull the front drive shaft, some guys take the tail gate off and put a net in its place or even a tonneau cover for less wind resistance, there would be a gift idea for you. A less agressive set of tires with narrow profile might be another, I would check with him first.

removing the tail gate actually reduces fuel mileage, its all in the airodynamics, just ask the mythbusters

A tonneau cover should increase mpg at highway speeds, I am suprised that a removed tailgate will not decrease wind resistance because I see garbage transport tractor/trailers with tail gates that are designed to swing into the trailer when empty (a big manufacturing expense) so they are more areodynamic when empty. If there is no noticable difference in mpg with a tailgate net, it at least would be a nice gift for your husband, I think he would appreciate it, but that's just my opinion.
