What's wrong with my car? How do I fix my brand new clutch?

So we bought a brand new clutch and we resurfaced the flywheel and a week later while driving the clutch went out. How can I fix my car? What should I look at?

When you replaced the clutch, did you replace the :

Pressure plate ?

Throwout bearing ?

Pilot bearing ?

check clutch fork ball joint for damage or lack of lubrication ?

Check the clutch fork for damage or wear ?

Clutch cable for damage or wear ?

Clutch hydraulic release cylinder for leakage / proper operation ?

When you resurfaced the flywheel is the measurement,,at,,or near,,the minimum thickness ?

Transmission input shaft damage,,seal leak,,?
